糖尿病的干细胞治疗——原理、证据与在日本就诊全解析

糖尿病并非只是”血糖偏高”,而是一种会随时间推移损害血管、神经、肾脏与眼睛的全身性代谢疾病。对许多患者而言,即使规范用药,病情仍在缓慢进展。近年来,自体脂肪来源间充质干细胞(ADSC)治疗作为再生医疗的一种新选择受到关注。本文由日本大阪Cell Grand Clinic的医师,从原理、证据与就诊流程角度进行说明。

什么是糖尿病的干细胞治疗?

干细胞治疗是采集患者自身的脂肪组织,分离并培养其中的间充质干细胞后,再回输体内的再生医疗方式。间充质干细胞具有抗炎、改善微循环、调节免疫等多种作用,研究认为这些作用可能有助于改善胰岛β细胞所处的环境以及慢性炎症状态。

为什么常规治疗会遇到瓶颈?

1型与2型糖尿病,哪种更适合?

2型糖尿病以胰岛素抵抗和慢性炎症为主,被认为更契合间充质干细胞的作用机制。1型糖尿病属于自身免疫导致β细胞受损,干细胞的免疫调节作用是研究的方向之一,但目前仍处于研究阶段,效果因人而异。是否适合需由医师结合病程与检查结果综合判断。

不仅是血糖:针对糖尿病并发症

干细胞治疗的一个特点是着眼于全身。对于糖尿病常见的末梢神经病变、血流障碍、肾功能下降等,通过改善血管内皮功能与抑制炎症,有望从整体上支持身体状态。当然,效果存在个体差异,并非对所有人都能保证同样结果。

证据与现状

关于间充质干细胞治疗2型糖尿病的临床研究正在全球范围内推进,多项报告提示其在安全性与血糖相关指标方面具有一定潜力。不过,现阶段仍缺乏大规模长期随机对照试验,应将其理解为”在规范治疗基础上的补充选择”,而非替代既有治疗。
