膝关节的干细胞治疗——非手术选择、证据与在日本就诊

膝关节疼痛,尤其是膝骨关节炎,是软骨随年龄与负荷逐渐磨损所致。当止痛药、玻尿酸注射与康复训练难以改善时,许多患者被建议接受人工关节置换手术。自体脂肪来源干细胞(ADSC)治疗作为一种不开刀的选择,正受到越来越多关注。本文由日本大阪Cell Grand Clinic的医师解说。

膝关节的干细胞治疗是什么?

采集患者自身脂肪中的间充质干细胞,经培养后注入关节腔。干细胞可通过抑制炎症、改善软骨环境、调节免疫、促进组织修复等多种机制,帮助缓解疼痛并改善关节功能。

为什么保守治疗会遇到瓶颈

非甾体抗炎药(NSAIDs)、玻尿酸与类固醇注射主要缓解症状,难以逆转已经磨损的软骨。干细胞治疗着眼于关节内部的炎症与修复环境。

证据与本院实例

2025年的多项荟萃分析提示,间充质干细胞在疼痛评分(VAS)与关节功能方面具有改善潜力。Cell Grand Clinic的一例膝骨关节炎(Kellgren–Lawrence 3级)病例已在国际期刊Cureus(2026)发表,报告显示治疗一年后疼痛VAS由8降至0、JOA评分由55升至95,MRI提示软骨与半月板改善(个案报告,效果因人而异)。

按分级看效果与细胞数量

一般而言,KL分级I–III级的改善潜力较高,IV级(重度)改善较为有限。研究还提示,单次治疗的细胞数量达到约1亿个被视为一个重要门槛。本院对细胞数量与品质进行标准化管理。
